What Annota is
Annota is a documentation assistance tool. It transcribes consultations and produces structured SOAP notes for your review and signature. Annota does not provide medical advice, diagnoses, or treatment recommendations. You remain the clinical authority for every patient encounter.
Your responsibilities
You must review and sign every note before using it in patient care. You are responsible for clinical decisions made on the basis of those notes. You must obtain appropriate patient consent before recording consultations, per the laws and professional standards of your jurisdiction.
Subscription and billing
Subscriptions renew monthly or annually until cancelled. Cancel anytime in settings, two clicks. Annual subscriptions are pro-rated and refunded on cancellation.
Liability
Annota's liability is limited to the subscription fees paid in the 12 months preceding any claim. You agree to indemnify Annota against patient claims arising from your clinical decisions.
Jurisdiction
These terms are governed by the laws of Malaysia. Disputes are heard in Malaysian courts.
